Regional Update: Care Connect services from OCH ended June 30, 2023


Olympic Community of Health’s work on the Care Connect WA, a program to support individuals in isolation for COVID-19, ended on June 30. Care Connect was a helpful resource during the acute phase of COVID-19, providing much needed resources to community members in isolation. It has been an honor to collaborate with our valued local partners and we celebrate our regional accomplishments in this work. OCH looks forward to incorporating the lessons learned, systems, and infrastructure built to work under future care coordination efforts.

From September 1, 2022 to June 30, 2023, 270 families were served, totaling approximately 733 individuals. OCH and our valued partners delivered 352 food and care kits for immediate relief and provided 245 orders of fresh food totaling just over $55,000 to families in isolation across Clallam, Jefferson and Kitsap counties, and to many of the Tribal nations in the region. 207 household assistance requests for mortgage, rent, and utility assistance, were paid out into the community, totaling nearly $195,000 in relief!





About 90% of clients who enrolled in the program requested a fresh food order. And little more than 75% of clients who enrolled in the program requested household assistance, most frequently requesting assistance with rent and mortgage. OCH is grateful to have provided supportive services to people who stayed home to stop the spread of COVID-19. A healthy community is a thriving community, and we are all stronger together.
