Introducing Care Connect Washington


Care Connect Washington has taken flight across the Olympic region!


Olympic Community of Health has contracted with the WA Department of Health to serve as the regional care coordination hub serving Clallam, Jefferson, and Kitsap counties for Care Connect WA, a program supporting individuals and families in quarantine and isolation for COVID-19.


Care coordination brings together local organizations, Tribes, and sectors to collaboratively create person-centered systems of care that ensure each client’s needs are met timely, easily, and compassionately. Care coordination tackles health issues that no single sector or Tribe can tackle alone. Care Connect WA is a FREE and low barrier program. Eligibility for enrollment in Care Connect WA is simple: (1) you must be in Washington State, AND (2) you must currently be positive for COVID-19. That’s all!


This program offers shelf stable food and hygiene kits, fresh grocery deliveries, up to $1500 in household assistance (subject to vendor participation), and referrals to other appropriate local resources as needed. Examples of bills that are covered by this program include:

In the first three weeks since OCH has launched the Care Connect WA hub, 16 families have been served, including 35 adults, 15 children, 7 cats, and 5 dogs! OCH and our valued contracted care coordination agencies have delivered 15 food and care kits for immediate relief and 10 days of fresh food to families in isolation and quarantine across Kitsap and Clallam counties.

If you or someone you know tested positive for COVID-19 and need food or other assistance to isolate, contact the COVID-19 Information Hotline at 1-800-525-0127, then press #. Language assistance is available.

The hotline hours are:

  • 6 a.m. to 10 p.m., Monday

  • 6 a.m. to 6 pm., Tuesday through Sunday and observed state holidays
